Andhra Pradesh

Eligibility & Procedure
• Dr NTR VaidyaSeva Health Insurance Scheme provides universal health coverage to BPL families

Application Form should accompany the following documents:
• Enumerated and photographed in White, Ration Card linked with Aadhar card and available in Civil Supplies Department database.

Quantum of Assistance:
• The scheme shall provide coverage for the services to the beneficiaries up to Rs.2.50 lakh per family per annum on floater basis. There shall be no co-payment under this scheme.

1. Benefit Coverage under of Universal Health coverage

• Out-Patient
Primary care is addressed through free screening and outpatient consultation both in the health camps and in the network hospitals as part of scheme implementation.
• In-patient
The scheme shall provide coverage for the 1044 “Listed Therapies” for identified diseases in the 29 categories – Are listed in website: www.ntrvaidyaseva.ap.gov.in

Package includes the following services:
• End-to-end cashless service offered through a NWH from the time of reporting of a patient till ten days post discharge medication, including complications if any up to thirty (30) days post-discharge, for those patients who undergo a “listed therapy(ies).
• Free OP evaluation of patients for listed therapies who may not undergo treatment for “listed therapies.
• All the pre-existing cases under listed therapies are covered under the scheme.
• Food and Transportation.

The scheme provides coverage for the procedures under following categories

Follow-up Services
Follow-up services are provided for a period of one year through fixed packages to the patients whoever require long term follow-up therapy in order to get optimum benefit from the procedure and avoid complications. Follow-up package for consultation, investigations, drugs etc., for one year for listed therapies were formulated by Technical committee of the Trust in consultation with specialists. Telangana

Government of Telangana provides free health facilities to families below poverty line through Aarogyasri, a unique health scheme being implemented through Aarogyasri Health Care Trust. This Health Scheme enables the poor families to avail quality medical treatment for serious ailments in a cashless manner, using medical infrastructure in both the private and public hospitals.

Enrollment: There is no separate enrollment, all the BPL families holding valid white ration card are automatically covered from the day of implementation of the scheme.

Eligibility: All poor families of the state of Andhra Pradesh, as defined by Civil Supplies Department of Government of Andhra Pradesh as BPL families, shall be eligible under this scheme. Eligibility card: The eligible families are provided with Below Poverty Line ration cards or Rajiv Aarogyasri Health Cards, herein after called eligibility cards.

Eligibility card for this scheme means:

i. White ration card;
ii. Antyodaya Anna Yojana (AAY) card;
iii. Annapurna card;
iv. Rajiv Aarogyasri Health card
v. TAP card
vi. RAP Card

Eligibility verification: The eligibility of beneficiary under the scheme shall be verified using Aarogyasri IT application or through any other means as decided by the Trust.

Excluded beneficiaries: Such of the beneficiaries who are covered for the “listed therapies” by other insurance scheme such as CGHS, ESIS, Railways, RTC etc., shall not be eligible for any benefit under this scheme.

Family: Family means members as enumerated and photographed on the Rajiv Aarogyasri Health Card or BPL Ration Card. The photograph or name indicated in the Health Card or BPL Ration Card will be taken as the proof for determining the eligibility of the beneficiary

Quantum of Financial Assistance: The scheme provides coverage up to 2.0 lakhs for each family per year. Rs.1.50 lakhs financial coverage on family floater basis i.e. if some amount is utilized by any of the family member the remaining amount will be available to the other members of the family. Plus an additional sum of Rs.50,000 buffer on recommendation of technical committee constituted by the Trust for deserving cases on an individual / family basis.

Other Benefits of Aarogyasri:
i. Food will be provided during hospitalization as part of treatment under the package.
ii. Transportation charges from patient mandal headquarters to the hospital at the rate of RTC fare will be provided by the hospital as part of package
iii. The patient should collect discharge summary from hospital, collect 10 days medication, collect transport charges and ascertain when to come back for follow-up. Having satisfied with services you shall sign the patient satisfaction certificate .You may seek help of MEDCO and Aarogyamithra in this regard.
iv. A round the clock call centre works with the help line No. 104. Call centre executives can answer both in Telugu and English languages. As it is a toll free number you need not pay any payment for making calls to this number. Call centre answers the queries.

Chief Minister’s Comprehensive Health Insurance Scheme

Eligibility & Procedure
• Every member of a family whose annual family income is less than Rs.72,000/- as certified by Village Administrative Officer

Karnataka

Eligibility & Procedure
• The assistance may be given directly to individuals or to institutions caring for the relief of distressed people.

Application Form should accompany the following documents:
• Applications for aid from the Fund for medical and or on poverty considerations should be supported by reports of Medical and Revenue Officers respectively. The Chief Minister’s satisfaction as regards the adequacy of proof in any case will be final.
• Beneficiary has to produce proof of utilization of the aid from the intended purpose. Cases in which such proof is necessary shall be decided by the Chief Minister.

Bengal

Eligibility & Procedure
• Should be a permanent resident of Bengal
• Distressed patients who have undergone treatment in the Govt/Non Govt Hospitals /Institutions and unable to bear the cost of treatment
• Patients who are not getting any reimbursement from employer/organization
• Should not be a beneficiary of Employees state Employees State Insurance scheme.
• Expenditure incurred prior to the date of receipt of Application to Chief Ministers Office is not eligible.
• Financial assistance can be availed only once for the same case once per year.

• If patient is admitted, all papers relating to the probable or actual expenditure incurred for the treatment including quotation , the tentative date of operation certificate from the concerned authority.
• Monthly income certificate from any one of the following officers Official Letterhead

c. Block Development Officer (BDO)
d. Group –A officer not below the rank of Jt. BDO/

• Required to submit No Objection Certificate (in case of Non-Blood related Donor) from the Health Department, Govt. of West Bengal. Salt Lake ,Kolkatta or Affidavit (in case of Blood related Donor) from the First Class Judicial Magistrate (Original/attested) if case of Kidney transplantation.

• For cases where treatment has to be undertaken, the money sanctioned would be paid to the concerned hospital/medical institution.

Quantum of Assistance: 
• For treatment costing up to Rs. One lakh in India, the quantum of assistance would be limited to a maximum of Rs. 30,000/- and for those over Rs. One Lakh it would be limited to a maximum of Rs. 50,000/- (ii)
• For handicapped person requiring artificial limbs etc. Maximum ceiling for limbs, cycling and other aids would be Rs. 50,000/- subject again to a fixed proportion of the cost of the item.
• No patient would be provided relief from this fund, a second time.

Madhya Pradesh

Eligibility & Procedure
• Should be a permanent resident of Madhya Pradesh
• Belongs to Below Poverty Line, and
• Only once to one member of a family
• The disease should be from the 20 listed diseases given in http://www.health.mp.gov.in/

a. The applicant has to give application in prescribed from to Collector of the District in the name of Director Public Health & FW.
b. The Collector / Sub Divisional Magistrate will give certificate of below poverty line & residential certificate and forward it to Civil Surgeon for certification of the disease.
c. The estimate of the recognized institution (Economy class ) shall be attached for giving grant . the application is forwarded to the Secretary, State Illness Assistance Fund

Odisha

Eligibility & Procedure
• The person should be a resident of odisha, and
• Annual income not exceeding Rs.40,000/- in rural areas and Rs.60,000/- for urban areas will be considered
• Either BPL card or Annual income certificate may be accepted as the eligibility criteria subject to verification of BPL card.
• Any person shall be assisted only once in a financial year.

Quantum of Assistance
• Assistance sanctioned will be limited to 1/3 rd of the estimated cost
• In case of (1) renal transplant, (2) cardiac surgery and (3) cancer, the maximum ceiling of assistance shall be Rs. 50,000/- for treatment within the state. Rs. 25,000 -2,00,000

Punjab

Eligibility & Procedure
• Families belonging to Below Poverty Line inder State Illness Fund through Punjab Nirogi Society.
• Every cancer patient except govt. employess and those having health insurance cover.

Application Form should accompany the following documents:

• Copy of Required Residence Proof
• Attested photocopy of Laboratory Report
• If ever taken financial help, attach copy of sanction
• Detailed estimate of cost of treatment after the date of application (with break up and tentative time schedule) from the hospital where the treatment is going on currently.
• Estimate proforma should not be older than 15 days from the date of Estimate dispatch.
• Two passport size photographs attested by concerned doctor.
• If belonging to SC/ ST, documentary evidence thereof.
• Self-declaration
• Diary Certificate by Civil Surgeon/ Medical Superintendent, if late by 7 days from date of Diary, then attach Certificate by respective Deputy Commissioner with reasons.

Note: Case will be considered under Mukh Mantri Punjab Cancer Raahat Kosh Scheme only after complete submission of above mentioned documents along with application proforma.

Quantum of Assistance
• MukhMantri Punjab Cancer RaahatKosh Society treatment of all cancer patients an amount of upto 1.50 lakhs is made available for treatment of every cancer patient except Govt. employees and those having health insurance cover.
• School children suffering from cancer are provided free treatment by Health Department

Eligibility & Procedure

Quantum of Assistance
• For Treatment in Government Hospitals 40% of Expenditure or a maximum of 100000
• For treatment in Private Hospitals 30% of Expenditure or a maximum of 60000

• Only general ward is considered for treatment. For better facilities if special ward/ cottage ward/ Private room/ semi delux room/ delux room used relief fund will not be given.

(ii) MukhyamantriAmrutum(MA) Yojana
• The total sum assured for the BPL Family is of Rs. 2,00,000 per family per anum on family floater basis. A unit of five members (Head of family, spouse, and three dependants) of family are covered under MA
• Services will be rendered by both empaneled private/public/trust hospitals.
• Rs. 300/- per hospitalization with a ceiling of maximum Rs. 3000/- per year will be reimbursed as transportation cost to the beneficiary

Assam

Assam Government issued notification for A health Assurance Scheme Atal Amrit Abhyan from February 2016.

Eligibility & Procedure
• Assam resident needs to get enrol himself/herself at authorised kiosks to become a member.
• Each beneficiary shall be issued a card
• Free for Below Poverty Line (BYL) Families
• For families having income Rs.5 lac or below Rs.100/- per year for each member of family will be charged.
• Card validity is for three years for beneficiary not belonging to below poverty line. A sum Rs. 100/- will be charged as premium for Non BYL member for renewal every year form second year onwards.
• All students studying in government schools are covered under this project by the education department Assam.

Quantum of Help
• Rs.2,00,000 per member per year
• There is provision of transport Allowance (Rs.300/- per visit) subject to a maximum of Rs. 3000/per year for an individual within the state (included under Rs. 2,00,000)
• For out of state treatment economy return by air fare for the person and for an attendant shall be provided.
• For out of state treatment Rs. 1000/- per day to a maximum of Rs. 10,000/- provided the number of days matches with the hospital length of stay and treatment days.
• There is provision for cashless treatment at selected hospitals for members.

Manipur

Manipur State Illness Assistance Fund (MSIAF)

Eligibility & Procedure
• Patient must be permanent resident of Manipur and has to furnish domicile proof of residence in Manipur.
• Patient should belong to “Below Poverty Line” family as notified by the State Government from time to time, and certified by an Officer not below the rank of Sub-Divisional Officer.
• Government employees are not eligible under this scheme. However, retired Govt. Employees are eligible, subject to fulfilling income criteria.
• Treatment should be from Jawaharlal Nehru Institute of Medical Sciences (JNIMS), Imphal; Regional Institute of Medical Sciences (RIMS), Imphal; or other Government Hospitals.

• Domicile Certificate
• Certificate A- in the case of patients who are not admitted to Hospital for treatment
• CertificateB – in the case of patients who are admitted to Hospital for treatment

• Financial aid so granted shall be a one-time grant only with an upper limit of Rs 1.00 (Rupees One Lakh) only.
